BioScrip Pharmacy Services, Inc. (“BioScrip”) is pleased to provide our customers with the convenience of an online pharmacy for ordering and re-ordering over-the-counter and prescription medications through a secure, user-friendly interface. Our website also includes tools and features designed to provide a useful resource for disease management information and to help track out of pocket medical expenses for you and your family.

BioScrip is committed to maintaining the privacy and confidentiality of our registered online users and visitors to our website (“you”). This website is designed to be used and accessed by users over the age of thirteen (13). Information We Collect

General. When you visit our website, either a guest or as a registered online pharmacy user, we may collect “non-personally identifiable information” from you. We only collect “personally identifiable information” from persons who provide us with the information we request in order to create and access an individual registered online pharmacy user account. Only individuals who create a registered online pharmacy user account will be eligible to utilize the full range of features and services available on our website, including our online pharmacy services.

Non-Personally Identifiable Information. “Non-personally identifiable information” is information that we collect from you that cannot be used to identify or contact you, such as your site browsing patterns or demographic information.

Personally Identifiable Information. “Personally identifiable information” is information that we collect from you such as your name, address, telephone number, date of birth, social security number, group health insurance member identification number, employer information, e-mail address, billing information, credit card number, non-prescription product interest and purchasing information, and prescription information. We will collect your personally identifiable information when you register and create an online user account and use our website or log onto your account.

How We Collect Information

Personally Identifiable Information. We collect personally identifiable information about you when you actively give us this information. Examples of this include when you become a registered user, respond to our surveys, give us feedback, and request information.

Non-Personally Identifiable Information. We also collect non-personally identifiable information from you by tracking your use of the our website, often through the use of cookies

Cookies. A “cookie” is a small data file that is sent to your browser from a web server that you access and stored on your computer’s hard drive. This cookie enables us to gather non-personally identifiable information and customer statistics about customer browsing. This data is aggregated and does not isolate the habits of a particular user. All Accounts. In the event that all of our business assets are transferred to or acquired by another company, your information may be one of the transferred assets.

Health Insurance. If prescription coverage is provided through your health insurance issuer or group health plan, your health plan, insurance issuer and/or third party administrator will have access to all of your information. We are not responsible for the use of your information by your health plan, insurance issuer or health plan administrator.

Manage Prescriptions for Family Members. While at the BioScrip web site, your information can be accessed through your username and password or, if you selected the “auto login” feature, through the unique identifier stored on your computer station.

As an individual registered user, you may also authorize access to certain portions of your account information by designated family members, caregivers, or other individuals involved in your care. This feature is specifically designed to simplify the prescription order process for families and other registered users who wish to link their accounts for ease of administration or other personal reasons.

In order to establish a linked account group, each individual registered user of the linked account group must agree to provide access to the other members of the account group by signing an authorization form. If you agree to provided access to your account information through a linked account group, another authorized group member will only be able to access your existing prescription numbers, order/refill history, and payment information. Specific medication information is not available through linked accounts. If you have questions regarding establishing a linked account group, please contact us.

Links to Other Web Sites

The BioScrip web site may contain links to other sites whose information practices may be different than ours. These links are provided as a convenience to our users and do not constitute affiliation with, an endorsement or sponsorship of the web sites or the parties that operate those sites. BioScrip is not responsible or liable for the information collection practices or content of such linked sites. Any information you provide to those third parties is not subject to the terms and conditions of this privacy policy. Visitors and registered users who link to other web sites through the BioScrip web site should consult the other sites’ privacy notices, as we have no control over information that is submitted to, or collected by, these third parties.

Security

Our web site allows you to order over-the-counter medications and prescriptions through Secure Socket Layer (SSL) technology. SSL technology provides for the secure transmission of information you give us over the internet

BioScrip uses technology based security controls to protect your personally identifiable information from unauthorized access, loss, misuse or alteration when it is in our possession. The personally identifiable information that we collect about you is stored in limited access servers, password protected and accessible by only a limited number of our personnel.
